2015 Potomac Health Foundation Annual Report

Potomac Health Foundation is happy to present the 2015 Annual Report.

Potomac Health Foundation was established in 2009 as a result of the merger between Potomac Hospital and Sentara Healthcare, Inc. Potomac Hospital used proceeds from the merger to establish the Foundation. The Foundation started its grant making in 2011. The mission of the Foundation is to improve the health of the community by increasing access to primary health care for the medically underserved, reducing the incidence of preventable diseases, and supporting innovation around emerging health needs.

The Foundation makes grants to community health programs serving individuals in eastern Prince William County, Virginia and adjacent communities in southeast Fairfax and north Stafford Counties. According to 2010 U.S. Census data, the community served by the Foundation is delineated by 13 zip codes and is comprised of more than 366,000 people. Needs assessments have indicated that within the region, there is significant unmet need and substantial room for improvement in health-related lifestyles, health status, and access to health services.

To date, the Foundation has given $27,910.923.60.
